On the history of surgical treatment of peptic ulcer disease

Despite significant changes in the conservative treatment of peptic ulcers over the past 35–40 years, it continues to hold a leading position among gastrointestinal disorders. The development and implementation of eradication therapy have not led to the expected reduction in the number of severe ulcer complications, which have been observed 1.5–2 times more often in Russia over the past 15–20 years.
The use of planned organ-sparing procedures resulted in a reduction in the incidence of complicated peptic ulcers in the 1970s and 1980s. However, the rejection of these procedures in the 1990s led to a 1.5–2.5-fold increase in the frequency of perforated and bleeding ulcers. It has been established that certain types of gastroduodenal ulcers are incurable; thus, a full resolution of the issue still requires surgery in some cases, meaning that the classical indications for surgical treatment of peptic ulcers should remain in effect.
The development and implementation of organ-sparing and minimally invasive procedures for the stomach and duodenum represent a progressive trend aimed at radically curing ulcer patients while minimizing damage. It can be assumed that the prospects for surgical treatment of peptic ulcers in the 21st century will largely depend on the improvement and introduction of advanced, physiologically compatible minimally invasive technologies.
